How to Submit a Sharing Request
Sharing requests can be conveniently submitted in your Member Portal. Sharing requests must be submitted within six months of the date of service; however, the sooner you submit a sharing request, the easier it is for Zion HealthShare to process and have the community share in your medical costs. For non-emergency medical services, especially surgeries, we recommend you contact Zion HealthShare or submit your sharing request before the date(s) of service.
To determine sharing eligibility, Zion HealthShare needs you to provide specific information as part of your sharing request. Be sure to complete as much of the sharing request form as possible and attach all relevant documents—including itemized bills and any relevant medical documents.
Review The Member Guidelines
The Member Guidelines explain the types of medical expenses that are eligible for sharing with the Zion HealthShare Community and the types that are not. Reviewing the Member Guidelines is the easiest way to determine whether your medical need will be eligible for sharing or if it is your responsibility as a member.
We specifically recommend that you review Pre-Membership Medical Conditions, Medical Expenses Eligible for Sharing, and Medical Expenses Ineligible for Sharing.
Present as Self-Pay Patient & Get Itemized Bills
Zion HealthShare keeps monthly contributions low for every member by sharing according to the Member Guidelines. To process your sharing request, we will need itemized bills for each service.
If you have insurance, you must disclose any insurance coverage to Zion HealthShare when you submit your Sharing Request. If you do not have insurance, tell your provider that you are a self-pay patient, and you need itemized bills with self-pay discounts.
Pay your IUA
The Zion HealthShare Community cannot share in a member’s medical expenses until their IUA has been met.
You can meet your IUA by paying your medical bills directly to your provider and submitting receipts to Zion HealthShare. When you do, ask for itemized bills for all costs. Submit these bills to your Member Portal so that we can confirm your IUA payment and begin the sharing process.
To make the process easier, you may pay your IUA directly to Zion HealthShare so that sharing for eligible medical expenses can begin right away. The IUA must be paid within six months of the date of service, or the medical expenses may become ineligible for sharing.
Sign a HIPAA Release Form and Work With Your Provider
To make an eligibility determination, Zion HeathShare may require medical records and other documents from your provider, as well as itemized bills for your eligible expenses. In order to receive medical records efficiently, there is a HIPAA Authorization Form you may need to sign so we can request your medical records for you. A HIPAA release also allows us to communicate with your providers to ensure a smoother sharing process. Please provide Zion HealthShare with your HIPAA release form as soon as possible and work with your provider so we can access the documents we need to share into your eligible medical expenses.

Sharing Request FAQ
What costs are eligible for sharing as part of a single sharing request?
Expenses related to the same medical condition may be shared under a single sharing request. Eligible expenses paid toward treatment of this condition will accumulate toward the member’s initial unshareable amount (IUA).
What should I expect after submitting my sharing request?
- A confirmation email with any additional instructions from Zion HealthShare. (In order to receive medical records efficiently, you may need to sign a HIPAA Authorization Form so we can request your medical records.)
- The determination of sharing eligibility for your sharing request within the Zion HealthShare community.
- If determined eligible for sharing, you will receive email updates about status changes, provider payments, reimbursements, and/or any missing information.
What documentation do I need before submitting my sharing request?
How long does it take Zion HealthShare to share in my medical expenses?
How do I submit a sharing request if I signed up for Zion HealthShare through an affiliate or my company?
How do I submit a sharing request if I have traditional insurance as well?
Please make sure that you submit a sharing request and all subsequent medical bills/statements through your Member Portal within six months of any medical need.
How to Set Up Your Member Portal
If you receive your membership through your employer or organization, this video will walk you through how to set up your member portal. All primary members should receive an invite to set up their member portal account. If you need immediate access to your member portal to submit a sharing request and have not received your invite, please contact Member Support at (888) 920-9466 for assistance.
Once you’re in your portal, you’ll see an option to invite the other adults on your membership.
How to Manage Membership Permissions in the Zion HealthShare Portal
What Are Membership Permissions?
Membership Permissions are a feature in the Member Portal that allows you to authorize or restrict another adult member in your household from taking certain actions on your behalf, based on the permissions you choose. This can include viewing, editing, or submitting sharing requests, as well as viewing or updating contact information.
Managing membership permissions in the Zion HealthShare Member Portal is simple once you know where to look. Follow the steps below to get started:
Step 1: Log In and Access Membership Settings
After logging into your Member Portal, you will see a menu on the left side of the screen. Click Membership located under Dashboard. This will take you to a page displaying membership details and a list of household members.
Step 2: Review Household Member Information
On the membership page, you’ll notice that all adult members have a message indicating their information has been redacted. If an adult household member hasn’t registered for the portal yet, you’ll see an Invite Dependent button next to their name.
Important: Each adult must have their own unique email address to access the Member Portal.
Step 3: Configure Permissions
To set permissions for a household member:
Click the small downward-facing arrow on the right side of their name.
Select Configure Permissions. A new window will appear with options including:
Allow someone other than an adult dependent member to view and edit their personal membership information.
Allow someone other than the adult dependent member to view and edit sharing requests.
Select the permissions you want to grant, then scroll down and click Grant Permissions.
Step 4: Sign the Authorization Document
After clicking Grant Permissions, an authorization document will open for signature:
At the top of the document, check the box confirming you understand and consent to using electronic records and signatures.
Click Agree and Continue.
If you’re signing as the member’s guardian, complete the optional guardian field. Otherwise, you can skip this step.
Add your signature in the final box, then click Finish in the top-right corner.
Step 5: Confirmation
Once completed, the selected adult household member will have permission to view and manage personal information and sharing requests within the Member Portal, based on the permissions granted.

A couple of things to keep in mind:
- Each adult (18 or older) needs their own unique email address to join the portal.
- Sharing requests can only be opened by an adult from their own portal account.
- If ever you change your email you will need to let us know so that we can reset, your login credentials.
- If you don’t have unique emails for everyone in your household, just give Member Support a call at (888) 920-9466 and we’ll help you out.
After your portal is ready, you can choose to give other household members permission to access your information or even open sharing requests on your behalf.

How to Use Virtual Cards for Pre-Payment
Zion HealthShare operates on a foundation of mutual trust and shared responsibility. This agreement clarifies our approach to pre-payments and virtual card usage, outlining how they impact your sharing privileges. By using Zion HealthShare’s pre-payment and virtual card services, you agree to adhere to the guidelines and responsibilities set forth in this document. Please read below to learn more about the proper use of your Zion HealthShare virtual card and pre-payments.
What are Pre-Payments?
Pre-payments allow members to submit eligible medical bills for consideration by the Zion HealthShare community before they incur the cost themselves. If deemed eligible for sharing, the community can share costs upfront, making it easier to manage larger medical expenses before the incident occurs. It is crucial to ensure all information submitted with a pre-payment request is accurate.
Issuance and Use of Virtual Cards
Upon approval of your sharing request, Zion HealthShare may issue a virtual card to facilitate payment at the time of medical service, or for medical expenses that have already occurred, provided an itemized bill has been submitted. Your Zion HealthShare virtual card is a temporary, unique card generated for time-of-service payments related to an approved sharing request. As a reminder, Zion HealthShare will only share in the related costs as outlined in the sharing request’s determination for sharing, any additional services will require review.
How Virtual Cards Work
Upon approval of your sharing request, you will receive a virtual card via email with a pre-loaded amount designated for that sharing request’s eligible medical expenses as outlined in the Member Guidelines. The card will expire after a set time limit. Any unused funds will revert back to Zion HealthShare. Bills eligible for sharing received after the time-of-service date should be emailed to needs@zionhealthshare.org or submitted through your member portal.
Proper Use of the Virtual Card
- Do not share your virtual card information with anyone unauthorized to use it.
- Use the virtual card only for the designated eligible medical expenses associated with the approved sharing request.
- Submit a payment receipt after using the virtual card.
Improper Use of Virtual Cards or Community Funds & Overpayments
If Zion HealthShare identifies an overpayment, a misuse of community funds, or improper use of a virtual card, you will be notified by email. Zion HealthShare defines improper use of a virtual card as using the card for any purpose not approved by Zion HealthShare. In a separate email, we will provide instructions on how to return those funds to the Zion HealthShare community. You will have fourteen (14) days from the notification date to return the full amount. Sharing will be paused during that fourteen (14) day period, and your household’s pre-payment and virtual card privileges will be suspended for one year if the funds are not returned. Once you return the funds in full, your household sharing will resume.
If the full amount is not returned within fourteen (14) days, the sharing restrictions mentioned earlier will continue, and all sharing requests submitted on or before the notification date will no longer be eligible for sharing. If the funds are not returned within thirty (30) days, your membership will be revoked.
Dispute Resolution
If you disagree with the findings about the funds owed to the community, you have up to thirty (30) calendar days from the notification date to explain your dispute in writing and send it to needs@zionhealthshare.org. During this time, all deadlines and sharing request holds will still be in effect. Your dispute will be reviewed, and you will be informed of the final decision and next steps by email.
Any payments returned to us will not be used for household sharing requests or applied to you IUA (Initial Unshareable Amount). Additionally, we reserve the right to recover these funds through various methods, such as involving collection agencies or taking legal action.
By participating in Zion HealthShare’s pre-payment and virtual card services, you acknowledge and agree to the terms outlined in this agreement. Please reach out to needs@zionhealthshare.org with any questions.
How to Refer Friends and Family
The Zion HealthShare Member Ambassador Program is our way of saying thank you to members who help grow our community by sharing their positive experiences. When someone you refer joins Zion HealthShare and remains an active member for 60 days, you’ll receive an appreciation gift. The program applies to new referrals who join on or after 11/24/2025.
Members can earn up to five rewards per household for the lifetime of the program.
Cómo Funciona
- Refer someone you know. Share Zion HealthShare with a friend, family member, or colleague.
- They enroll. The referred household joins Zion HealthShare and becomes an active member.
- You earn a reward. After their first 60 days of active membership, you’ll receive your appreciation gift.
- Rewards are typically issued 2-4 weeks after verification.
Appreciation Gifts
Referring members may receive up to five total rewards per household for the lifetime of the program.
1st Referral: Zion HealthShare Swag Store Credit
2nd Referral: National Parks Annual Pass
3rd, 4th, & 5th Referrals: $100 credit toward your next IUA (credits may be stacked)
Eligibility
- Only active Zion HealthShare members in good standing are eligible to participate and receive rewards.
- The referring member must remain active at the time the reward is issued.
- The referred household must be new to Zion HealthShare and must not have any previous enrollments.
- The referred household must remain active for 60 days and two full months of contributions.
- Program participation is voluntary and not a condition of membership.
Referral Submission
To qualify for a reward, a referral form must be submitted within 90 days after referred household’s membership start date.
The form must include:
1. Referring member’s full name and email (as listed on their Zion HealthShare membership)
2. Referred household’s name, email, and ZIP code
Company Referral Rules
Referrals involving companies follow special guidelines:
(a) A referred company counts as one referral, regardless of the number of households that enroll.
(b) The company must remain active for 60 consecutive days.
(c) At least two households must be enrolled under that company for the referral to qualify.
Household Limit
Only one reward will be issued per referred household, regardless of how many individuals in that household enroll. Each Zion HealthShare household is eligible for a maximum of five total rewards throughout the lifetime of the Member Ambassador Program.
Program Availability
The Member Ambassador Program is not available to residents of:
Washington (WA), Montana (MT), Massachusetts (MA), Maryland (MD), New Mexico (NM), or Pennsylvania (PA).
Referrals from or for households in these states are not eligible for rewards. Zion HealthShare reserves the right to verify residence status.
Members are not permitted to actively solicit or market Zion HealthShare. The referral incentive is intended solely for sharing with family and friends in a personal capacity.
Ineligible Participants
The following individuals and entities are not eligible to participate or receive rewards:
1. Zion HealthShare employees, Board members, and their household members
2. Registered affiliates and their household members
3. Employees and affiliates of Planstin and Zion HealthShare Marketing Organizations, and any affiliates or representatives of those organizations
Verification and Fraud Prevention
All referrals are subject to verification. Zion HealthShare reserves the right to withhold or revoke rewards for any duplicate, fraudulent, incomplete, or unverifiable referrals.
Referrals must be for separate, unrelated households. Self-referrals or referrals within the same household are not eligible.
Only the first valid referral form submitted for a household will be considered. Duplicate submissions will not result in multiple rewards.
If a referral is submitted before the referred household has enrolled, the household must complete enrollment within four (4) weeks for the referral to be reviewed. Referrals that remain unregistered after four weeks will be closed and require resubmission to be considered.
Reward Fulfillment
Rewards are issued 2 to 4 weeks after the 60-day verification period. Rewards have no cash value and cannot be exchanged or transferred. Zion HealthShare is not responsible for lost or undeliverable shipments or delays by third-party delivery services.
Program Changes
Zion HealthShare reserves the right to change, suspend, or discontinue the program or its terms at any time without prior notice. Fulfillment processes may also be adjusted to comply with IRS regulations and organizational policies.
Participation may be reportable for tax purposes. Members are responsible for reporting rewards as income if required by law.
How to Withdraw Your Membership
Members (including a spouse or adult child) may withdraw their individual membership, completely or from a specific household membership, at any time. To avoid being charged for the next billing cycle, membership withdrawal requests must be submitted at least 2 business days in advance.
All requests may take up to the following business day to be processed, including those that take place on a holiday or weekend. The membership will remain active until the end of the household membership’s current billing cycle. Zion HealthShare cannot backdate withdrawals and therefore does not prorate withdrawals or provide refunds.
Only the primary member may remove other members from the membership. Once a membership is withdrawn any open sharing requests and all future sharing requests will no longer be eligible for sharing (unless the membership is reinstated before thirty (30) days). Members should plan accordingly. Requests for membership withdrawal must be submitted through the Member Portal Admin Request Form.
Note: If your membership is administered through your employer or any entity other than directly through Zion HealthShare, you must reach out to that entity in order to withdraw.
How to Transition to Medicare
Zion HealthShare memberships are available to all individuals under the age of 65. Most individuals above the age of 65 qualify for free or low-cost medical care through Medicare. We recognize the importance of this transition and aim to provide supportive information on navigating the shift to Medicare.
Zion HealthShare Membership and Age Limit
When a primary member or spouse reaches the age of 65, they are no longer eligible to continue membership with Zion HealthShare. Their membership will be withdrawn on the billing cycle in the month in which they turn 65. As you or your spouse approach age 65 you may have questions about how this transition works. Individuals under the age of 65 may remain members of the Zion HealthShare community if they choose. If the individual turning 65 is the primary member, those wishing to remain on the membership will need to become a primary member. Qualifying sharing requests submitted before age 65 will remain eligible for sharing and will be processed as quickly as possible. As per our member guidelines:
Medical incidents, services, and expenses that occur prior to the members age-out at sixty-five (65), will have up to thirty (30) days after their membership end date to submit a sharing request. All other time limits will remain in effect including:
- Itemized bills and statements must be submitted within six (6) months of the date of service, and
- The IUA must be paid within six (6) months of the date of service, or they become ineligible for sharing.
Understanding Medicare
Medicare is a government health insurance program primarily for individuals aged 65 and older, though eligibility may also extend to those with disabilities, End-Stage Renal Disease (ESRD), or ALS. Enrollment can be automatic or require active sign up, often tied to Social Security Benefits. Part A and Part B coverage is automatic for those applying for Social Security benefits at least 4 months before turning 65. Those who want to receive Medicare benefits, but do not want retirement benefits right at age 65, will need to actively register. Click here to learn more about enrolling with Medicare.
Parts of Medicare
Medicare comprises several parts, with the most common being:
Part A: Hospital Insurance
- Inpatient hospital care
- Skilled nursing facility
- Lab tests, surgery
- Home health care
- Hospice
Part B: Medical insurance
- Doctors and other health care providers’ services and outpatient care
- Durable medical equipment
- Home health care
- Some preventive services
Part C: An optional addition covering wellness programs, adult day-care services, transportation to doctor visits. Also known as Medicare Advantage
Part D: Another optional addition for prescription drug coverage

Important Considerations Before Transitioning to Medicare
Transitioning to Medicare can raise many questions. Here are some key points to consider:
- Pre-existing Conditions: Medicare cannot deny coverage for pre-existing conditions.
- Choosing a Medicare Plan (Part C & D): Original Medicare (Parts A & B) covers a large portion of medical expenses, but not everything. Consider enrolling in a Medicare Advantage Plan (Part C) for additional coverage, or a Part D plan for prescription drug coverage. Explore your options and choose a plan that best meets your needs and budget.
- Medicare Costs: Medicare has premiums, deductibles, and copays. The Medicare Plan Finder tool can help you estimate costs for different plans.
- Network of Providers: Some Medicare plans have specific networks of doctors and hospitals. Make sure your preferred providers are included in the plan you choose.
- Timeline for Enrollment: There are specific enrollment periods for Medicare. Late enrollment penalties may apply so it is important to understand the Medicare enrollment timelines. Click here to learn more about avoiding Medicare penalties.
